Description:

Consumer Services receives and responds to complaints against health care providers and determines whether complaints warrant investigation.

INTAKE PROCEDURE
Complaints must be filed in writing. A patient records release will be requested. Consumers may contact the Consumer Call Center for a Uniform Complaint Form or access it on the internet site.
